{"id":21530,"date":"2018-01-29T12:21:06","date_gmt":"2018-01-29T12:21:06","guid":{"rendered":"https:\/\/digileaders.com\/?p=21530"},"modified":"2019-01-17T09:46:52","modified_gmt":"2019-01-17T09:46:52","slug":"five-golden-rules-responsible-cloud-adoption-nhs","status":"publish","type":"post","link":"https:\/\/digileaders.com\/five-golden-rules-responsible-cloud-adoption-nhs\/","title":{"rendered":"Five golden rules for responsible cloud adoption in the NHS"},"content":{"rendered":"<p class=\"p1\"><span class=\"s1\"><b><i>As NHS Digital endorses placing health data in the public cloud, we ask what responsible cloud use looks like in the NHS.<\/i><\/b><\/span><\/p>\n<p class=\"p2\"><span class=\"s1\">For some time we have been calling on the NHS to move to <a href=\"http:\/\/bit.ly\/2E19jtj\"><span class=\"s2\">the cloud as a cheaper, more secure alternative<\/span><\/a> to on premise and outsourced IT. We therefore warmly welcome <a href=\"https:\/\/www.digital.nhs.uk\/article\/8486\/NHS-and-social-care-data-off-shoring-and-the-use-of-public-cloud-services\"><span class=\"s2\">NHS Digital\u2019s recently issued guidance<\/span><\/a> that endorses placing health data in the public cloud. The guidance encourages health and care organisations to harness the potential of cloud, thereby becoming more secure and providing better efficiencies, and allowing NHS Trusts to do \u2018more for less\u2019.<\/span><\/p>\n<p class=\"p2\"><span class=\"s1\">It is hoped that this official cloud endorsement will also unlock a wave of innovation in the NHS, especially in areas like IoT solutions. These solutions will generate a wealth of new and enriched data, that can then further inspire yet more specialist solutions that will themselves deliver new insights and help reduce costs and improve outcomes. There are a number of factors that need to be considered here:<\/span><\/p>\n<ul class=\"ul1\">\n<li class=\"li2\"><span class=\"s1\">Security and trust \u2013 Patients and citizens have a right and an expectation of privacy \u2013 especially from governments and big businesses that might seek to abuse these datasets.<\/span><\/li>\n<li class=\"li2\"><span class=\"s1\">Standards and interoperability \u2013 devices and applications need to be integrated to deliver real value, providing seamless access to whatever data and insights are needed wherever, whenever and however.<\/span><\/li>\n<li class=\"li2\"><span class=\"s1\">Data analytics and intelligence \u2013 Big data analytics can examine massive datasets of genomic and patient data to identify trends, provide insights and derive intelligence.<\/span><\/li>\n<li class=\"li2\"><span class=\"s1\">Data gravity \u2013 pooling valuable data on a secure, trusted and connected platform that is harnessed by a vibrant ecosystem of specialist solutions.<\/span><\/li>\n<li class=\"li2\"><span class=\"s1\">Cloud Computing \u2013 Underpinning all of this is cloud computing. IoT devices feed data to the cloud, where the capacity exists to store and process it all. It is in the cloud where the data is pooled for analytical application to mine for insights to turn into intelligence which can then be accessed remotely by mobile devices to empower both clinicians and patients.<\/span><\/li>\n<\/ul>\n<p class=\"p2\"><span class=\"s1\">With cloud as the underlying enabler for so many other areas of innovation, the new guidance from NHS Digital is most welcome. It comes at a time though when there has been a growing trend towards multi-cloud strategies and a realisation not only that <a href=\"https:\/\/diginomica.com\/2018\/01\/03\/2018-cloud-comes-age-customer-centricity-becomes-key\/\"><span class=\"s2\">no single cloud is appropriate for all workloads<\/span><\/a>, but also that <a href=\"https:\/\/diginomica.com\/2017\/02\/10\/lock-starts-surpass-security-main-cloud-concern\/\"><span class=\"s2\">cloud lock-in represents an increasing threat<\/span><\/a>. <\/span><\/p>\n<p class=\"p2\"><span class=\"s1\">In addition, various organisations and publications (examples <a href=\"https:\/\/diginomica.com\/2018\/01\/22\/nhs-endorses-use-public-cloud-including-us-companies-covered-privacy-shield\/\"><span class=\"s2\">here<\/span><\/a> and <a href=\"https:\/\/www.theregister.co.uk\/2018\/01\/22\/nhs_data_offshore_us_privacy_shield\/\"><span class=\"s2\">here<\/span><\/a>) have expressed surprise that NHS Digital\u2019s guidance on cloud should include off-shoring to US data centres, as long as the companies are covered by the Privacy Shield agreement. The Privacy Shield agreemen allows firms to sign up by self-certifying to the US Department of Commerce. However, the EU&#8217;s data protection agencies said in December that they have &#8220;significant concerns&#8221; about it, and even the European Commission has admitted a number of improvements are needed. Issues on the US side include the lack of a permanent ombudsman to oversee the deal, how compliance with the scheme will be monitored and how firms making false claims will be weeded out. Indeed, <a href=\"http:\/\/www.thinkdigitalpartners.com\/news\/2017\/10\/31\/eu-us-privacy-not-merry-go-round\/\"><span class=\"s2\">Privacy Shield is being challenged from all sides and its future is anything but guaranteed<\/span><\/a>.<\/span><\/p>\n<p class=\"p2\"><span class=\"s1\">As ever though, the NHS Digital guidance came with a number of caveats. Many of these emphasised that responsibility for things like security, privacy and compliance cannot be outsourced. Indeed, along with the new guidance, NHS Digital provided links for more details on its cloud security good practice guide, its cloud risk framework, its data risk model and its cloud security overview.<\/span><\/p>\n<p class=\"p2\"><span class=\"s1\">To help address the key challenges of adopting the global public cloud platforms and help outline what responsible cloud use looks like, we have drawn up the following golden rules that NHS Trusts would be wise to follow:<\/span><\/p>\n<h4 class=\"p2\"><span class=\"s1\">Security:<\/span><\/h4>\n<p class=\"p2\"><span class=\"s1\">Consider using a government-grade provider for your most critical workloads and patient Identifiable data sets \u2013 those providers with adequate assurance levels and with connectivity to HSCN. Caldicott principles mandate levels of protection for patient identifiable data. With HSCN providing secure connectivity, data transfer via the public internet should be avoided \u2013 especially given the heightened level of cyber activity, as we saw with Wannacry. Trusts also need to be prepared for the DoH 2017\/18 data and security guidance, which the NHS will have to measure from this April (see <a href=\"https:\/\/www.gov.uk\/government\/uploads\/system\/uploads\/attachment_data\/file\/675420\/17-18_statement_of_requirements_Branded_template_final_22_11_18-1.pdf\"><span class=\"s2\">here<\/span><\/a>).<\/span><\/p>\n<h4 class=\"p2\"><span class=\"s1\"><b>Flexibility:<\/b><\/span><\/h4>\n<p class=\"p2\"><span class=\"s1\">Adopt a multi-cloud approach to ensure that you get the right cloud for each of your workloads. Avoiding cloud lock-in and maximising flexibility with multi-cloud and you\u2019ll not only find the best fit for each workload in the cloud, but also be able to cater for applications that were never designed to run in the cloud.<\/span><\/p>\n<h4 class=\"p2\"><span class=\"s1\"><b>Sovereignty:<\/b><\/span><\/h4>\n<p class=\"p2\">Keep your data in the UK. Despite NHS Digital\u2019s guidance mentioning Privacy Shield, the framework\u2019s future is far from assured and you may need to repatriate your data if, like Safe Harbor, it is overturned. There are few scenarios in which you should need to take data off shore in the first place, so why take the risk?<\/p>\n<h4 class=\"p3\"><span class=\"s1\"><b>Compliance:<\/b><\/span><\/h4>\n<p class=\"p3\"><span class=\"s1\">Most organisations are going to have their work cut out preparing for GDPR, so avoid complications that could compromise your compliance. For example, the global cloud providers maintain the right to export your data anywhere in the world, without notice, irrespective of your choice of processing region, which is contrary to both GDPR and NHS Digital\u2019s published guidance. <\/span><\/p>\n<h4 class=\"p3\"><span class=\"s1\"><b>Future Proofing:<\/b><\/span><\/h4>\n<p class=\"p3\"><span class=\"s1\">As we move towards a paperless, more integrated and more intelligent future, we will be expected to query ever larger data sets with ever greater speed and efficiency (potentially triggering <a href=\"https:\/\/bit.ly\/2EGEXeD\"><span class=\"s2\">egress charges and other hidden costs<\/span><\/a>). Ensuring that your data sets and workloads are in close proximity to those of your peers, of your key application providers and of key third parties (such as Genomics England with the rise of genomic medicine) will become increasingly advantageous.<\/span><\/p>\n<p class=\"p2\"><span class=\"s1\">We see a bright, innovative future for the NHS as well as a way of not only improving efficiency and of in part alleviating its constant financial pressures, but also of improving patient outcomes.
